A request arrives. It gets recorded as a need, becomes a line in the training plan, then a supplier, a budget, and two days out of the calendar for forty people. Rarely does anyone check what happened before the request.
Was the standard ever made clear? Does the process allow the behaviour being asked for? Have the tools, the workload or the role changed? Ask those first and the answer is often that training was never the right intervention. Ask them afterwards and the money is already spent.
A strategic Training Needs Assessment is how that check gets made — properly, and before the budget is committed.
